Please contact your insurance carrier for questions about your individual health plan.
Our office runs your insurance to confirm that your policy is active at time of service. We can estimate your out of pocket costs if you have a deductible plan but we can not guarantee any pricing for you. Your insurance company dictates how much money you owe once your claims are processed. Claims can take between 30-90 days to process from each date of service. We strongly advise all patients to keep reference numbers when speaking with their insurance plans in case claims process differently than you expected. We bill as outpatient physical therapy in an office setting.
If you disagree with a bill you receive from our office, please check your explanation of benefits from your insurance company. We bill you based on how your insurance company processed your claim. If you do not have access to your explanation of benefits you need to contact your insurance company and request copies be mailed to you or have an online portal set up for you to access these important documents.

WE DO HAVE SELF PAY RATES FOR INITIAL EXAMINATIONS AND FOLLOW UP VISITS. PLEASE CALL THE OFFICE TO INQUIRE ABOUT PRICING.
Unfortunately we do not participate with Keystone Healthplan East.
You would need to contact your primary care physician's office or insurance plan to find out where your capitation site is for physical therapy. Being that we are a private practice, we are not a capitation site.
We do not participate in any Medicaid Plans including Keystone First, United Healthcare Community Plan & Aetna Better Health.
We do not participate with Tricare/Humana as the primary insurance. We do accept these insurances as secondary insurance to Medicare.
